The Pharmacy Service at Reina Sofía Hospital in Córdoba has a robust history of developing technological tools to enhance medication management across various contexts. Several years ago, the hospital introduced TTPharmacy, a program designed to improve the traceability of medication dispensation. Building on this success, the hospital plans to launch a new project called Gemer Salud after the summer.
Gemer Salud, a collaborative effort involving pharmacists, doctors, and IT specialists, aims to provide certainty about the real therapeutic value of treatments. This initiative will support future decision-making for healthcare professionals and provide reliable information to patients, fostering a more direct communication channel with their specialist pharmacists.
With around 220 patients daily, including those with chronic conditions, immunosuppression, biological treatments, and oncology patients, Gemer Salud is poised to be a significant step forward in the hospital’s digital transformation. The tool aims to improve clinical management of medications, tracking them from request to real-life patient outcomes for a better evaluation of treatment impacts.
Gemer Salud will optimize therapeutic decisions by providing real-world clinical data on treatment safety and effectiveness. It also enables patients to communicate with their reference pharmacists via chat, allowing them to report how they feel, note side effects, or clarify administration queries.
The tool has been well received by other clinicians. While it has been piloted with specific patient subgroups, the goal is to extend its availability to all patients seen by the External Patient Unit.
The development team includes pharmacists, doctors, and IT engineers from Reina Sofía Hospital, alongside external advisors from other centers. Doctors are crucial to the program, as it will streamline medication requests and monitoring, advancing the digitalization of this healthcare process.
For patients, Gemer Salud will offer detailed information about their conditions and treatments, reducing misinformation and the need to seek unreliable online sources.
Gemer Salud activates when a doctor requests a high-impact medication for a patient. The request is validated by the pharmacy service and a multidisciplinary committee, after which the patient can collect the medication and optionally enroll in the application. The app provides treatment-specific information, chat access, medication reminders, a suggestion box, FAQs, and quality of life surveys.
Set to officially launch at the Andalusian hospital after summer, Gemer Salud promises to significantly enhance clinical medication management in hospitals, improving the safety, efficiency, and quality of treatments by combining advanced technology with clinical expertise.
